Our Care Management program helps older adults and their family members determine long-term care needs and access the resources available. The task of determining whether an older relative is safe and happy at home is never easy. Health issues, daily functioning, and isolation are just some of the concerns that family members struggle with. Since 1996, Visiting Nurse Health System has provided care management to older and disabled adults in metro Atlanta. We provide these services to over 3,800 individuals and families annually.
Care Management is available to anyone concerned about an older relative. Families may wish to request an in-home assessment and evaluation by professional care managers or want help locating the right services. We can offer ongoing follow up and monitoring for families trying to care for elderly parents or relatives from a distance. We ensure that each assessment draws from the experience of a wide variety of disciplines, from physical therapy to nursing, social work and more, reflecting our years of experience in care management.
