To refer a patient to hospice, please download the referral form, print and fax the completed form to 404-848-7965. If you have questions, call us at 404-869-3000.

Visiting Nurse Health System continues to set the standard for quality home healthcare, care management, and hospice care in Georgia.  Our interdisciplinary hospice team provides medical, emotional and spiritual support to patients and their families.  We also work closely with the patient’s primary physician and other healthcare professionals to ensure a smooth transition to hospice.
