Between visits from their home health nurse, patients daily check their own health indicators including blood pressure, heart rate and weight with the help of a state-of-the-art, easy-to-use device installed in their home. With the click of a button, the information is sent to a specially-trained clinician at our office who reviews it. If the clinician notices any significant or troubling changes, she notifies the patient’s regular home health nurse and, if needed, their doctor for additional follow up.
